The Indian Air Force (IAF) has revealed that the accidental firing of a BrahMos combat missile into Pakistan in March 2022 was caused by the failure to disconnect the combat connectors of the missile before movement. The IAF conducted a Court of Inquiry to determine the cause of the incident.
