WA Coroner Demands Closure of Troubled Youth Unit After Teen’s Death

A coroner has called for the closure and investigation of Unit 18, a youth detention unit at Casuarina prison in Western Australia, after the death of Indigenous teenager Cleveland Dodd. Cleveland, 16, was found unresponsive in his cell early on October 12, 2023, and died a week later in hospital. He became the first juvenile to die in a WA detention facility. Coroner Phil Urquhart stated the unit must close urgently and a special inquiry launched to examine its creation. "No child in detention deserves to be treated in the way Cleveland and the other young people in Unit 18 were treated at the time he decided to end his life," Urquhart said. The report found Cleveland endured excessive solitary confinement, isolation, and poor access to health, education, and even running water. His cell had a known hanging point that was unrepaired. Despite multiple requests, he was denied counselling. Staff described Unit 18 as a "war zone" with chaotic conditions. Before his self-harm incident, Cleveland spent only 1-2 hours out of his damaged, unfurnished cell daily. The coroner added that his death was not a result of human error by staff but long-term system failures. Fifteen adverse findings and nineteen recommendations were made, including creating a forum to discuss youth justice management. The WA government says improvements since Cleveland’s death include plans for a new purpose-built youth detention center within three years.
